Mission

Miles of Hope Breast Cancer Foundation is a non-profit Foundation with a mission to provide funds to support programs for people affected by breast cancer in the Hudson Valley.

Grant Guidelines

The Following criteria are to be utilized for the evaluation of all applicants:

• Must be undergoing treatment for breast cancer.

• Must have exhausted all resources available to the individual. (health insurance, public assistance ect.)

• Must be a resident of one of the 8 eligible counties, but you do not need to be a US citizen. (New York State counties include Columbia, Dutchess, Greene, Orange, Putnam, Rockland, Ulster, and Westchester)

• Must be requesting for a service or item that is not covered by insurance.

• A health professional associated with the patient care must make the recommendation to the program.

• Money awarded will not exceed $1000 per award. Consideration will be made on a case by case basis.

• Must meet income eligibility requirements or have special circumstances to be considered.

A consultation with a representative of the Miles of Hope Breast Cancer Foundation is required.

All applicants must complete a DCCAP Intake form and provide a photo I.D., valid Social Security Card, Birth Certificate and proof of income.
